About Hospice

Hospice is a program of care for people who are considered to be in the last phase of life. Northfield Hospice’s team of nurses, social workers, chaplains and hospice volunteers work with patients, their families and healthcare providers to provide comfort, dignity and peace of mind during this time of transition. This care is provided wherever the hospice patient calls home, including nursing homes and assisted living facilities.

About Home Care

Home Care provides skilled nursing visits to area residents who want to have state-of-the-art care, but find it difficult to leave their homes. Northfield Home Care’s highly trained staff of nurses, home health aides and therapists provide an array of medical and personal-care services, so individuals can continue to enjoy an independent lifestyle without compromising well-being. As more people choose to “age in place” rather than move to a nursing home or assisted living, Home Care helps individuals remain in their own familiar environment for as long as possible – without compromising their medical or personal care.
